Treatment of acne is based on the type and severity of the pimples. For many teenagers with acne there is no one effective remedy. Individuals respond to acne medication differently and need different treatment approaches. The best acne medications are based on their ability to inhibit the multiplication of the bacterium Propionibacterium acnes and to cause clearing of the keratin at the occluded pore of the pimple.

The pimple is an enlarged, sebum-filled gland whose opening is partially or completely obstructed by keratin from the hair follicle. Acne medications belong to four categories. Among the first of those groups are Tretinoin and Isotretinoin. These are related to Vitamin A and act on the keratin plug that blocks the sebum filled gland. They loosen the keratin material and release the sebum from the pimple.

The second group of acne drugs are the antibiotics. Acne is associated with infection of the sebum producing sebaceous gland by a bacterium called Propionibacterium acnes. This bacterium multiplies in sebum and alters its nature. Therefore antibiotics such as Tetracycline, Doxycycline, Erythromycin and Clindamycin have the potential to control the bacterial infection. The antibiotics have to be continued for several weeks and in some cases for months. They should be discontinued only when there are no more fresh crops of pimples or when they start producing adverse effects. Benzoyl peroxide are also antibacterial. They often form the initial line of therapy along with Tretinoin.

The third group of drugs is the anti-inflammatory drugs. When the pimple enlarges with accumulation of sebum, the walls of the gland get thinned out and rupture, producing seepage of the sebum into the adjoining tissue. This causes inflammation and the acne progresses to a chronic form. The best medications to control acne-related inflammation are the corticosteroids. For severe inflammatory acne, oral Prednisolone is required. Inflammation can also be prevented by corticosteroids. Triamcinolone, injected into a large pimple, especially cysts, causes closure of the cyst cavity and healing with minimum inflammation. This mode of treatment is a blessing to teenagers who are prone to develop severe acne.

The fourth group of drugs are the ones that control sebum production by the sebaceous gland. Estrogens, taken as oral contraceptive pills, help decrease sebum and thus control acne. Other drugs, too, have a similar effect. Low dose corticosteroids and Isotretinoin also have a role in the decrease in sebum produced by the glands.

Newer drugs such as Adapalene and Azelaic acid are very effective against acne. Azelaic acid is versatile as it has antibacterial, anti-inflammatory and anti-keratin effects.

The best acne medications may need to be restricted in acne usage when side effects occur. Benzoyl peroxide can cause irritation of skin.
